Unlike many programs that require major insurance changes or doctor referrals, GUIDE is designed to work alongside a person's existing care team. There is no need to switch doctors or leave your current health system.

Who Qualifies for GUIDE?

Eligibility is based on two key factors:

  • The person living with dementia must have traditional Medicare (not Medicare Advantage).

  • They must be living in the community, including their home or a memory care unit (not a nursing home or on hospice).

Dr. Taylor notes that a formal dementia diagnosis is not required to apply. If the family caregiver suspects cognitive decline, HōttoCare can assist with the assessment process.

"That's our bread and butter. If there's any suspicion of cognitive impairment, they can apply. We can help with the diagnosis."
